Communities In Schools of Appalachian Highlands will be providing a limited number of students an opportunity to participate in the CIS Back-to-School Clothing Giveaway.
Individuals/groups/churches will choose a student’s form and then purchase appropriate school clothing items for the beginning of the upcoming school year. Your child will not be identified by name but by a number only.
Acceptance into the program is completely dependent on the participation of local individuals, businesses, and churches. If your child is sponsored, a letter will be mailed to your home address in July indicating when and where you can pick up your child’s items. If your address changes between now and July, please let us know by emailing info@cisofah.org with updated information.
Additionally, you will be asked to sign a parent/guardian consent form giving Communities In Schools of AH permission to provide individual services for your student/students should the need arise throughout the school year. This will be required in order to receive items through the Back-to-School Clothing Giveaway.
Use the links on the right hand side to fill out a form for each of your students. Please make sure to click the link for the school division that your student(s) attend. If you need a paper copy, please let your school Student Support Coordinator know. Please note that elementary and middle school students will receive 2 outfits, socks, and underwear and the high school students will receive shoes, a hoodie, socks, and underwear.
Please fill out one form per student, making sure you provide the most accurate size information. When in doubt, SIZE UP. We cannot exchange/return items once you pick them up.
